Required Documents

The documents that you will need to bring with you to your appointment will depend on whether or not you are in possession of your primary passport.

  • Confirmation from UKVI/foreign Embassy that you have submitted your application (if available);
  • A printed copy of your travel itinerary showing that you have confirmed booked travel within 5 working days (if available);
  • Valid government issued photo ID  (such as a driver’s license or a passport of another nationality);
  • Proof of U.S. citizenship, e.g. expired U.S. passport, CRBA, U.S. birth or naturalization certificate (if available);
  • One U.S. passport sized (2×2 inch) photograph – see photo requirements for information or check our SlideShare presentation.  U.K. size photographs are not accepted;
  • A completed “Statement in Support of Application for a Second Passport” (Click to download);
  • The non-refundable application fee for this service is $145.00.  Fee information is available here; and
  • Form DS-11.  Before you begin, see our YouTube video for instructions on completing the form. Complete the form online and print.  Do not sign it.  You are required to sign the form before a consular officer.

If you have submitted your passport to UKVI or a foreign Embassy and you have confirmed travel plans please first contact them to request the return of your passport.  If they are unable to return your passport to you, please refer to the instructions above “I will not have my passport on the day of my appointment”.

Note you will be required to show evidence that your travel is within 5 working days of your appointment with the U.S. Embassy.
